I wneud cais, llawrlwythwch y ffurflen gais briodol isod a dilyn y cyfarwyddiadau mewn print trwm ar dop y ffurflen.

Dylai eich cais gynnwys:
- 1 x Ffurflen Monitro Amrywiaeth
- 2 x gopi o'ch CV mwyaf diweddar (1 x copi os ydych yn gwneud cais ar–lein)
- 1 x copi o'ch llun(iau) castio diweddaraf (yn gymwys i berfformwyr yn unig)
- Tâl* (os yn gymwys)
